This policy explains how Delos Longevity (“Delos,” “we”) handles information collected through this website. It does not cover the medical records and health information we hold as your healthcare provider, which are described in our separate Notice of Privacy Practices.
What we collect
- Information you send us. When you use our contact form, we receive your name, phone number, email address, your area of interest, how you heard about us, your message and your communication preferences.
- How you reached the site. Your browser stores details such as the page you first landed on, the referring website, and campaign or advertising click identifiers. This information is sent with your form if you submit one, so we can understand which marketing is working.
- General location. When a form is submitted, we may record an approximate location (city, region and country). We do not keep your IP address with your form.

Our website form isn’t designed for medical information. Please keep your message general and share health details with Dr. Gurrala at your visit.

Who we share it with
We use service providers to run this website and manage inquiries, including website hosting and security (Cloudflare, including its Turnstile spam protection) and customer communication software (HighLevel). They may process information on our behalf. We do not sell your personal information.
Text messages and email
You can opt out of text messages by replying STOP, and out of marketing emails by using the unsubscribe link. Mobile opt-in information is not shared with third parties for their own marketing.
